Medical Records

If you have had imaging studies such as CT scans, MRIs, PET scans, etc., please bring the films with you. It is an important part of establishing your treatment plan, so please bring your actual films or disc to your initial clinic visit (not just the reports).  Please make sure that your referring physician has sent all of your relevant medical records prior to your appointment. Our fax number is 877-991-8954. It is very important that we have these to ensure appropriate and complete care.
